Job Title: Physician Assistant (PA-C) | Senior Community Rounding Provider

Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

We're seeking a PA-C who wants to play a central role in helping patients navigate complex healthcare needs while receiving care directly in the communities they call home.

Our providers serve assisted living communities, memory care settings, and long-term care environments throughout the Twin Cities Metro. Rather than seeing patients in isolation, you'll work closely with nurses, caregivers, families, therapists, and physicians to create continuity, improve outcomes, and help patients receive the care they need at the right time.

What You'll Be Doing

• Provide on-site primary care and rounding services throughout assisted living, memory care, and long-term care communities.

• Perform assessments, chronic disease management, preventative care, and follow-up visits.

• Develop and adjust individualized care plans.

• Coordinate care with physicians, nurses, therapists, caregivers, and family members.

• Support transitions of care and help reduce avoidable emergency room visits and hospitalizations.

• Ensure continuity of care across a diverse patient population.

Qualifications

• Active Minnesota Physician Assistant (PA-C) license in good standing.

• Experience in geriatrics, family medicine, internal medicine, home health, hospital medicine, post-acute care, or related settings preferred.

• Strong communication skills and confidence working collaboratively with interdisciplinary teams.

• Valid driver's license and reliable transportation.
