Job description

JOB SUMMARY: The Client Services Coordinator is responsible for all functions of the Pharmacy Connect of Southwest Virginia Program, which assists people of all ages to apply for free medications from participating pharmaceutical companies. The area to be served is Wise, Lee, and Scott counties and the City of Norton. The Client Services Coordinator will also function as an assistant to the Care Coordination department.


JOB RESPONSIBILITIES:

  • Accepts referrals for services from the elderly, non-elderly, their families, and the community
  • Assists with the application process by inputting patient data into the Pharmacy Connection software program
  • Provides assistance and direction to applicants in securing necessary documentation for program eligibility as required by each pharmaceutical company
  • Handles the large volume of telephone calls to and from clients and their medical providers which are necessary for determining eligibility or for problem solving
  • Processes pharmacy re-orders in a timely manner
  • Maintains client files with current and up-to-date information
  • Make home visits as needed to assist those applicants who cannot come to the MEOC office to apply
  • Assists with training for MEOC staff about the Pharmacy Connect program and the application process
  • Attends functions within the service area to educate and promote Pharmacy Connect, as well as other MEOC programs as needed
  • Prepares, maintains, and submits data, reports, or other statistical information as needed in a timely manner
  • Provides assistance and support as needed to other partners within the Pharmacy Connect of Southwest Virginia Partnership
  • Acts as a responsible advocate for the elderly of Planning District 1
  • Identify service and support needs and develop resources to meet those needs.

  • Maintain spreadsheets for program and funding reports.
  • Assist with developing and implementing policies and procedures for the operation of Care Coordination department in compliance with Virginia Department for Aging and Rehabilitative Services (VDARS) and other funding sources.
  • Provide direct counseling or make appropriate referrals for individuals in need of insurance assistance.
  • Perform required record keeping, reporting and program monitoring.
  • Assist with fundraising.
  • Attend agency staff meetings as well as Care Coordination team meetings as required.
  • Enters and/or documents everything in Peerplace, Virginia’s No Wrong Door Tool.
  • Assist clients with emergency services, working closely with the Director of Care Coordination and/or Care Coordinators.
  • Provide back-up coverage for front desk phones as needed.
  • Assist with all Care Coordination calls
  • Performs such other duties as may be required by the Director of Care Coordination or the Executive Director of Mountain Empire Older Citizens, Inc.

ESSENTIAL FUNCTIONS:

QUALIFICATIONS:

  • High School Diploma required

  • Must be able to drive, use computers, maintain detailed records, and relate well to the staff and clients
  • Ability to work independently and meet deadlines is essential
  • Ability to manage multiple priorities in a fast-paced environment
  • Must have knowledge of geographic areas and public benefits available
  • Knowledge of community resources, long term support and funding systems
  • Criminal background checks conducted by the Virginia State Police and Virginia Department of Social Services Child Protective Services Central Registry
  • Ability to handle confidential information with discretion and professionalism

WORK SCHEDULE: Monday through Friday, 8:30 a.m. to 4:30 p.m. and additionally as needed
