Are you passionate about providing compassionate care to those in need? Join our dedicated hospice team as a Certified Nursing Assistant (CNA) / Home Health Aide (HHA) and bring comfort, dignity, and support to patients and their families during life’s most important moments.

About the Role:
In this vital position, you will deliver personalized care in the patient’s home or facility setting—assisting with daily living activities, monitoring health changes, and offering emotional and physical support. Whether it’s helping someone bathe, preparing meals, or simply being a comforting presence, you’ll be the heart of our care.

What You’ll Do:

  • Assist with bathing, grooming, dressing, toileting, and personal hygiene.
  • Change linens, wash laundry, clean living areas, and maintain a safe environment.
  • Measure and report vital signs, intake & output, and observe/report health changes.
  • Assist with feeding, hydration, ambulation, and patient positioning.
  • Administer basic treatments such as enemas, heat/ice packs, and non-sterile dressings.
  • Support medication adherence as directed by nurses or physicians.
  • Prepare meals in accordance with dietary needs and assist with nourishment between meals.
  • Offer companionship, conversation, and emotional support.
  • Accompany patients to medical appointments or errands as needed.
  • Provide care for pediatric patients or disabled family members when applicable.
  • Document care and communicate effectively with the nursing team.
  • Assist with post-mortem care as needed in hospice settings.
