Job description

Outpatient RN Clinical Care Specialist 

Mooresville, NC | Full-Time | $36-$40/hr + Performance Bonus

Ascend Health and Wellness is a fast-growing outpatient medical practice specializing in hormone optimization (BHRT), weight loss medicine, peptide therapy, and preventative care. Our model blends personalized medicine with efficient systems and a high-touch patient experience. We have been dedicated to providing top-tier healthcare services for over 4 years. Our commitment to patient-centered care and innovative wellness solutions has earned us the trust and loyalty of our community. 
This is not a traditional RN role. We are hiring a high-performing Clinical Care Specialist to operate as an extension of the provider—managing triage, patient care, communication, and clinical coordination in a fast-paced, protocol-driven environment with nuanced cases. You will play a direct role in patient outcomes, retention, and overall experience.
What You’ll Do

•         Manage high-volume patient communication (phone + portal) with efficiency and professionalism
•         Triage concerns, determine what requires escalation vs. independent handling, and act decisively within scope
•         Review charts, labs, and patient history to support clinical decision-making and provider workflow
•         Coordinate refills, lab orders, and treatment follow-ups under provider guidance
•         Educate patients on treatment plans, injections, and lifestyle modifications
•         Monitor patient progress and proactively identify risks to outcomes or retention
•         Maintain precise, timely documentation
This role is NOT a fit if you:

•         Prefer step-by-step direction over independent decision-making
•         Are uncomfortable making clinical judgment calls within established protocols
•         Struggle with high communication volume or fast-paced environments
•         Prefer highly structured or slower-paced roles
Compensation & Benefits

  • $36-$40+/hour based on experience and clinical capability + up to 5% quarterly performance bonus based on measurable KPIs including patient satisfaction and retention, response timeliness to patient inquiries, clinical documentation and task completion.

Schedule

  • Monday–Friday, in person, approximately 8:30am–5:00pm. Friday clinical hours are shorter.
  • Schedule flexibility may be available based on performance and role ownership.
Requirements

•         Active North Carolina RN license
•         Experience with IM/SQ injections and patient education
•         Experience managing patient communication via EMR/portal systems
•         Strong critical thinking and ability to follow clinical protocols
•         Ability to operate independently with minimal oversight
•         High attention to detail and accountability
Preferred

•         2+ years outpatient medical care in primary care, urology, OBGYN, hematology, endocrine, or concierge medicine experience.
•         Comfortable managing patient communication independently
•         Demonstrated clinical judgment (not just task execution)
