Position Overview 

The Home Health Aide (HHA) provides non-medical in-home care and supportive services to clients in accordance with an individualized written care plan. This role focuses on assisting with activities of daily living (ADLs), promoting safety, offering companionship, and ensuring comfort while adhering to agency policies and consumer care plans. 

This PRN position offers flexible, as-needed hours and is ideal for HHAs seeking meaningful, one-on-one client care in a home-based setting. 

Key Responsibilities 

Personal Care & ADL Assistance 

  • Assist with bathing, grooming, dressing, toileting, oral hygiene, and hair care 
  • Provide safe transfers, ambulation, and mobility assistance using walkers, wheelchairs, or gait belts as needed 
  • Reposition clients to promote comfort and help prevent skin breakdown 
  • Observe, document, and report changes in client's condition to supervising staff 

Nutrition & Meal Support 

  • Prepare meals according to dietary guidelines and care plans 
  • Assist with feeding as needed 
  • Monitor and document food and fluid intake 

Companionship & Consumer Support 

  • Provide emotional support and meaningful social interaction 
  • Encourage independence, dignity, and client's choice 
  • Accompany clients to medical or community appointments, as assigned 

Household Support & Errands 

  • Perform light housekeeping, including laundry, changing bed linens, and cleaning kitchen and bathroom areas 
  • Complete errands such as grocery shopping and pharmacy pickups, as needed 

Compliance & Training 

  • Maintain active Home Health Aide certification or required training per state regulations 
  • Complete annual in-service and mandatory training 
  • Follow all care plans, safety protocols, and HIPAA/confidentiality standards 

Requirements: Qualifications & Requirements 
  • Active Home Health Aide (HHA) certification or proof of required training per state regulations 
  • Must be at least 18 years of age 
  • Physically able to perform job duties, including lifting, transfers, and extended periods of standing or walking 
  • Dependable, professional, and consumer-focused 
  • Ability to read, write, speak, and understand English 
  •  Valid Driver's License Reliable transportation to and from assignments 
  • Smartphone required for Connecteam work-related duties
