We are seeking a dedicated Registered Nurse (RN) Field Supervisor to join our passionate team. In this role, the RN will supervise, mentor, and support Certified Nursing Assistants (CNAs), Medication Aides (MAs), and Companions, in line with the policies and procedures set by A Place at Home and the state of Texas

The Registered nurse reports to the administrator and is expected to assist A Place At Home-River Oaks with nursing assessments, employee training, and any necessary administrative tasks to ensure that A Place At Home- River Oaks remains compliant with the Texas Department of Health. The registered nurse ensures quality service provision and appropriate supervision.

The registered nurse is responsible for working with other disciplines to ensure client needs are met, which includes, but is not limited to, the assessment process, the creation and maintenance of the plan of care, the coordination of services, and follow-up assessments within 90 days of the initial assessment and as needed in a change of client condition.

Essential Job Responsibilities:

• Provides excellent customer service skills and ability to deal tactfully with clients, staff, and community.

• If directed by the administrator, makes the initial evaluation visit and regularly re-evaluates the client's needs.

• Initiates the plan of care and makes necessary revisions, keeping the administrator updated on the revisions.

• Informs the administrator, physician, and other relevant personnel of changes in the client's condition and needs.

• Implements and ensures compliance with agency policies and procedures.

• Assists the administrator with assessing the competency of caregiver staff to promote quality, continuity, and safety of client care.

• Supervises, educates, and coaches other personnel as directed by the administrator.

• Ensures proper maintenance of records in compliance with local, state, and federal laws.

• Ensures compliance with all local, state, and federal laws.

• Identifies and participates in agency performance improvement activities.

• Stays informed of changes in the field of home care and overall healthcare to share with appropriate personnel.

• Promotes home care referrals in the healthcare community.

• Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times.

Qualifications:

Active RN License (Required)

Clinical experience or home care experience, preferred.

Minimum of one (1) year supervising experience, preferred.

Case management skills, preferred.
