Job Title: Weekend Registered Nurse: Classification: Non-Exempt (Part-time 30 hours per week)


Summary/Objective

The Weekend Registered Nurse (RN) is a vital member of the interdisciplinary team, responsible for providing continuity of care to patients in both homecare and hospice settings during Friday, Saturday, and Sunday coverage. The Weekend RN works directly with patients, communicating and collaborating on an ongoing basis with the full clinical team to address patients' changing needs. This position supports weekend coverage needs, reduces reliance on overtime, and helps maintain timely, patient-centered care across the weekend schedule.

Accountability

The Weekend Registered Nurse is accountable to the Clinical Manager.

Essential Functions

  • Works Friday, Saturday, and Sunday, 10-hour shifts, for a total of 30 hours per week.
  • Provides weekend RN coverage for patients in homecare and hospice settings.
  • Covers weekend on-call responsibilities during the scheduled 10-hour shifts.
  • Interprets the Agency's philosophy and corporate objectives related to both homecare and hospice to patients, their families, and community groups.
  • Performs all assessments, treatments, and documentation according to Agency policies and standards, including State and Federal regulations.
  • Develops, implements, evaluates, and modifies clinical services for patients under a physician's plan of treatment.
  • Engages in the hospice philosophy and provides patient-centered care.
  • Utilizes an Evidence-Based Chronic Illness Management approach to improve homecare patient outcomes.
  • Demonstrates proficiency in OASIS assessment and documentation.
  • Manages and coordinates the evaluation and admission of hospice patients as assigned.
  • Collaborates with the interdisciplinary team to provide clinical services according to best practices.
  • Documents interdisciplinary communication consistently.
  • Instructs patients and family members on the plan of care to help them assume responsibility when appropriate.
  • Assists in referring patients to other community resources as needed.
  • Consistently meets productivity guidelines, on-call responsibilities, and timely documentation requirements.
  • Maintains professional competence and enhances knowledge through continuing education.
  • Provides additional weekday coverage only when absolutely necessary and as requested by the Agency.

Qualifications

  • State License: Must have a current N.H. State Registration as a Registered Nurse (RN) in good standing.
  • Certifications: CPR Certified must be within 6 months of hire.
  • Training: Must have completed Hospice Training to make hospice visits.
  • Experience: Previous recent experience in a medical office, home health, hospice, or related clinical setting is required.
  • Transportation: Must have access to an operable motor vehicle, a valid NH driver's license with a safe driving record, and proof of car insurance.
  • Skills:
  • Excellent interpersonal communication and organizational skills are required.
  • Computer skills are strongly preferred.
  • Must be able to read, write, and speak English fluently.
  • Ability to pay close attention to detail and follow instructions.
  • Work Requirements:
  • Must be able to work Friday, Saturday, and Sunday, 10-hour shifts, for a total of 30 hours per week.
  • Must be available to cover weekend on-call responsibilities during scheduled 10-hour shifts.
