Aging Life Care Manager
Make a difference while also having the flexibility to control your schedule! Join our Complex Case Care Management & Special Needs Travel team and use your skills to greatly enhance the quality of life of seniors and other adults who are physically or cognitively challenged.
HOURS
A full-time position with some flexibility. The right candidate must be available to work some weekends, holidays or "on-call" as necessary for client safety. Hours will increase as case load increases.

EDUCATION
Masters in Social Work or RN, CMC Preferred
EXPERIENCE
Eligible for certification by the National Academy of Certified Care Managers
Require at least 5 years of RN experience in eldercare, LPN must have at least 10 years of experience in eldercare, a ARNP, Director of Nursing or Charge Nurse experience preferred. All must have 2 years of experience in Care Management, Social Work or Hospice is preferred. Candidate must have strong written and oral communication skills. A candidate must be a seasoned geriatric care specialist and have experience working with individuals with Alzheimer's disease or forms of dementia.
Experience with Jewel Code Care Complete or similar Care Management Programs preferred
AGING LIFE CARE MANAGEMENT RESPONSIBILITIES
Preparation of health & wellness assessment and recommendations, Planning, coordinating and implementing the care plan for each client, Supervise recommendations from the assessment and Care Management meetings, Coordinate client's physician, dental and other medical appointments, Supervise medical issues including medications and be present at doctor visits, Communicate with and support family members and other responsible parties, Prepare weekly schedules of planned and completed activities
- Chart all pertinent client information, legal level noting, Participate in the recruitment, orientation, evaluation and retention of service providers, Advocacy; obtain restraining orders, testify in circuit court and at the State's attorney level, testify as an expert witness, Must have reliable transportation to and from the client's residence
MARKETING / INTAKE RESPONSIBILITIES:
Take potential client intake calls to determine needs, align them with appropriate services and complete paperwork to start services. Attract referrals and inquiries to build care management services. Contact potential and existing healthcare referral sources on behalf of Silver Wings Care Management. Promote and position Silver Wings Care Management programs and services to medical professional, civic, community and Alzheimer's related groups in a manner that illustrates and reflects Silver Wings Care Management quality of care.
QUALITIES:
Independent worker, self-starter, organized, consistent follow-through, calm under pressure, leadership, planning and organizational skills, ability to train others, loyal, committed and passionate about quality care. Must be able to work out of your home. You must have a complete home office including smart phone, internet access, fax, scanner, copier, shredder and a dedicated office with a door.
SKILLS AND EXPERIENCE REQUIREMENTS:
Working knowledge of aging process to include diagnosis, psychosocial issues, medical procedures and medications; Knowledge of physical, emotional, social, leisure and spiritual needs of clients; Supervisory skills; Well developed communication skills (both verbal and written); Interpersonal communication skills; Thorough understanding and use of medical terminology.
COMPENSATION
Paid per hour on billed time depending on skills, certification and experience.
