Nurse Practitioner – Center for Healthy Aging

CHRISTUS St. Vincent | Santa Fe, New Mexico

CHRISTUS St. Vincent is seeking a compassionate, experienced Nurse Practitioner to join our Center for Healthy Aging in Santa Fe, New Mexico. This is an outstanding opportunity to provide meaningful, relationship-centered care to older adults in a supportive outpatient setting focused on helping patients maintain their independence, function, and quality of life.

At the Center for Healthy Aging, care is guided by the nationally recognized 4Ms FrameworkMentation, Medications, Mobility, and What Matters—ensuring a patient-centered approach that prioritizes the factors most important to healthy aging and overall well-being.

Opportunity Highlights

  • Full-time outpatient practice
  • Flexible schedule:
    • Monday–Friday, 8-hour workdays, or Four 10-hour workdays per week
  • Patient volume of approximately 16+ patients per day
  • Limited call responsibilities: Primary Care call rotation by phone only with nurse triage support
  • Collaborative, team-based care environment dedicated to the unique needs of older adults

Position Responsibilities

The Nurse Practitioner will provide comprehensive outpatient care for an aging patient population, including:

  • Conducting annual wellness visits and preventive health screenings
  • Evaluating, diagnosing, and treating acute and chronic conditions
  • Managing common geriatric concerns, including:
    • Dementia and cognitive disorders
    • Osteoarthritis and chronic pain conditions
    • Urinary incontinence
    • Multiple chronic medical conditions
  • Ordering and interpreting diagnostic tests and laboratory studies
  • Developing and managing individualized treatment plans
  • Collaborating closely with physicians, specialists, caregivers, and interdisciplinary team members
  • Supporting patient goals while promoting independence, safety, and quality of life

Qualifications

  • Nurse Practitioner certification required
  • Minimum of two years of Nurse Practitioner experience
  • Active New Mexico Nurse Practitioner license, or ability to obtain
  • Passion for caring for older adults and delivering patient-centered care
