About the Opportunity:

We are seeking an innovative and experienced Director of Nursing to lead our clinical nursing team and drive excellence in patient care across Affinity Health Center. This is a unique opportunity to shape the future of community health by building robust chronic disease management programs, strengthening care coordination, advancing clinical quality initiatives, and empowering a diverse team of healthcare professionals to deliver exceptional, patient-centered care to our underserved community.


Key Responsibilities:


Clinical Leadership Team Development

  • Provide leadership through the Nursing Supervisor to staff RNs, including performance management, scheduling and staffing optimization, professional development planning, and departmental budgeting
  • Support and develop the Medical Assistant team through the MA Supervisor to improve clinical workflows
  • Supervise Community Health Workers to expand outreach initiatives and strengthen SDOH (Social Determinants of Health) screening and support
  • Lead the Clinical Quality Coordinator in addressing chronic disease gaps and preventive care initiatives


Program Development Care Coordination

  • Design and oversee comprehensive chronic disease management programs that improve patient engagement, satisfaction, and health outcomes
  • Lead patient education initiatives and integrate remote monitoring technologies
  • Manage transitional care programs, including: post-hospital discharge follow-up, coordination with home health services and reduction of avoidable hospitalizations and ER visits


Quality Improvement Population Health Management

  • Provide leadership for clinical quality improvement initiatives aligned with HEDIS and UDS performance metrics and value-based care requirements
  • Drive population health strategies across patient panels
  • Collaborate with clinical teams to analyze quality metrics, implement evidence-based interventions, and monitor progress toward quality benchmarks


Operational Excellence

  • Optimize supply chain and inventory management across sites
  • Implement and sustain Lean 5S systems for efficient clinic operations
  • Collaborate with interdisciplinary teams to advance quality metrics and care coordination strategies


Qualifications:


Required

  • Active RN license in South Carolina (or willingness to obtain SC RN License)
  • 5–7+ years of registered nursing experience
  • 3+ years of proven clinical leadership/management experience
  • Strong knowledge of:
  • Quality metrics
  • Care coordination
  • Interdisciplinary team leadership


Preferred

  • Experience in case management and/or chronic disease programs
  • Background in supply chain or inventory management
  • Pediatric nursing experience, including familiarity with the Vaccines for Children program
  • Experience with clinical quality improvement and population health initiatives
