Job Description

WHAT YOU’LL DO

As a home care registered nurse (RN), you will ensure hospice patients are comfortable during their end‑of‑life journeys and forge compassionate, meaningful connections with the people in their lives.

You will:

  • Provide comfort‑focused in‑home nursing care for patients with prognoses of six months or less
  • Work on a team of interdisciplinary hospice clinicians, including LPNs, aides, chaplains, social workers and physicians
  • Serve as an advocate for each patient and family in your care, ensuring their goals of care are communicated and prioritized by each member of the care team
  • Deliver basic hospice education and training to patients, families and caregivers

WHERE YOU’LL WORK

You will spend most of your time caring for hospice patients wherever they call home, whether that means a private residence or an assisted living/skilled nursing facility.

You will typically make 4‑6 visits each day.

Once a week, you’ll visit the local program office to meet with an interdisciplinary team, including social workers, physicians, chaplains, and aides, who collaborate to provide holistic support for the patients and families we serve.

WHAT’S EXPECTED FROM YOU

  • An associate or bachelor's degree in nursing
  • A valid RN license
  • At least one year of hospital nursing experience
  • Reliable transportation
