Job description

A skilled nursing and rehabilitation facility in New York is seeking an experienced Director of Admissions to oversee the full admissions process and support census stability. This role is ideal for a strong admissions leader who understands hospital referrals, payer requirements, and how to manage smooth transitions into long-term and post-acute care.

Responsibilities:

  • Oversee all admissions from referral through admission

  • Coordinate with hospitals, case managers, discharge planners, and referral sources

  • Review clinical documentation to ensure appropriate placement

  • Manage insurance verification, authorizations, and payer requirements

  • Track referrals, conversion rates, and census trends

  • Work closely with nursing, social services, and administration to ensure seamless transitions of care

  • Serve as a point of contact for residents and families during the admissions process

  • Maintain compliance with state, federal, and facility admissions policies

Qualifications:

  • Prior admissions leadership experience in a skilled nursing facility required

  • Strong knowledge of Medicare, Medicaid, and managed care admissions

  • Experience working with hospital discharge planning teams

  • Excellent communication, organization, and follow-up skills

  • Ability to manage high referral volume in a fast-paced environment
