About the job Physical Therapist
Central Health Management, Inc. is seeking a full time Physical Therapist for a Monday - Friday 8 hour shifts providing care for patients in their home in Southwest Virginia.
Provide physical therapy services to patients under appropriate guidelines.
โ€ข Implement and conduct treatment as outlined in treatment plan.
โ€ข Treats clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments and procedures within the plan of care.
โ€ข Monitor patient's response to treatment and modify treatment as indicated in accordance with physician's orders.
โ€ข Interpret and communicate evaluation findings to patients, family and other staff members.
Qualifications:

  • Active and Valid Physical Therapy Licensed in Virginia
  • Valid Driver's license
  • BLS/CPR certified
  • Prefer Home Health experience, not required
