• Conduct thorough assessments of clients to identify their unique social, emotional, and environmental challenges, utilizing structured interviews and validated assessment tools.
• Design and implement individualized service plans that outline specific goals and interventions, regularly evaluating progress and making necessary adjustments.
• Provide compassionate counseling and ongoing support to individuals and families undergoing crises related to substance abuse and mental health.
• Facilitate client connections to community resources, such as housing assistance, mental health services, and rehabilitation programs.
• Deliver crisis intervention strategies, conducting risk assessments and developing safety plans tailored to each client's needs.
• Collaborate with multidisciplinary teams and community organizations to coordinate comprehensive services and advocate for clients’ needs.
• Engage in outreach initiatives to promote awareness of agency services and foster community partnerships.
• Participate actively in case conferences, team meetings, and professional development opportunities to enhance service delivery and ensure continuity of care.

You will primarily engage with individuals battling substance abuse and related challenges, providing essential support to help them overcome barriers and access necessary recovery resources. Our clients come from diverse backgrounds and often face multiple socio-economic hurdles. • A Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Master Social Worker (LMSW) in Colorado.
• Demonstrable skills in communication, cultural competence, and problem-solving.

Ideal candidates will possess at least two years of experience in a social service setting, particularly in case management or counseling roles focused on addiction and recovery. Familiarity with trauma-informed care and understanding socio-economic justice issues will be valuable in this position.
