Job Summary
The RN Float position is a flexible, field-based role that supports both home care and home health clients. The role requires travel within and outside of Philadelphia to provide exceptional nursing care, ensuring compliance with all clinical and documentation standards.
Key Responsibilities
  • Conduct SOCs within 48 hours of referral date.
  • Provide comprehensive and individualized nursing care to home care and home health clients.
  • Perform Resumption of Care visits and ensure a seamless transition back to services.
  • Conduct discharge visits, ensuring patients are transitioned appropriately.
  • Deliver skilled nursing visits, including wound care, medication administration, and patient education.
  • Complete all visit documentation and submit it within 24 hours of the visit.
  • Maintain accurate, up-to-date patient records in compliance with agency policies and regulatory requirements.
  • Accept all assigned visits and provide care as scheduled.
  • Adhere to all federal, state, and agency regulations and standards.
  • Maintain a high level of professionalism and provide excellent patient care.
  • Attend required training sessions and team meetings as needed.
Qualifications
  • Valid RN license in the state of Pennsylvania.
  • Minimum of 1 year of nursing experience (home care or home health preferred).
  • Strong organizational and time-management skills.
  • Ability to work independently and handle a diverse caseload.
