Job description

JOB DESCRIPTION: LICENSED VOCATIONAL NURSE - CLINICAL SUPPORT STAFF

REPORTS TO: RN Supervisor/Director of Patient Care Services

QUALIFICATIONS: Is a graduate of an accredited practical nurse or vocational nursing program Is currently licensed as a Licensed Practical Nurse or Licensed Vocational Nurse in California. Complies with accepted professional standards and practice. Possesses and maintains good physical stamina and mental health. Has presented a pre-employment physician's health clearance including a negative TB skin test and/or other tests as required by organization policy.

JOB DUTIES AND RESPONSIBILITIES

1. Assists in the assessment of patient's need, plan, implements and documents nursing care under the direction of RN Supervisor or Director of Nursing

2. Performs tasks based on the Nurse Practice Act; maintains agency compliance with Federal and State regulations.

3. Informs the physician and family members of the changes in patient's condition and needs.

4. Under the direction of the RN Supervisor, LVN promotes education for patient's families and patients; and participates in Case Conferences; participates in revising plan of treatment, problem list and/or care plan; assists in ordering supplies and equipment needed by the patient as requested by PCG or shift LVN.

5. Completes incident reports as needed per agency policy.

6. Maintains familiarity with case load's special routine for the patient, to facilitate care for patient and better relationship with patient's family; assists the RN by visiting patients to do regular "check-ins" to ensure all patient's needs are adequately met by agency..

7. When providing LVN patient care coverage: Follow the Plan of Treatment as ordered by the physician. Give complete report of patient to incoming personnel to ensure continuity of care e.g., condition, intake output, prn medication, and unusual symptoms. Promote patient safety at all times. Administer medication, monitor for adverse reaction. Notify MD for any abnormal findings and deviations from baseline. Implement new MD orders. Complete all necessary charting on each worked shift.

This is a PER DIEM position. You will be placed on our roster for your specified city coverage list. As cases becomes available, we will contact you to check your availability and confirm your interest in accepting the assignments.
