Description

Join Pilgrim Place, a mission-driven senior living community in Claremont, where compassion, teamwork, and person-centered care guide all we do. We are seeking a dedicated Neighborhood/Household Nurse (LVN) On Call (AM or PM Shift) to join our team in a on-call role.

In this role, you'll provide hands-on nursing care, support a resident-directed culture, and serve as a teacher and role model for neighborhood/household staff, ensuring high-quality, individualized care for every resident.

What You'll Do
  • Administer medications and treatments safely and according to plan of care.

  • Observe residents' physical and emotional condition and report changes to the Nurse Supervisor (RN), Director of Nursing, or attending physician.

  • Collaborate closely with the Nurse Leader to ensure regulatory compliance and best practices.

  • Support a person-centered, resident-directed environment that fosters independence and choice.

  • Serve as a mentor and role model for neighborhood/household staff, promoting professional nursing practices.

  • Provide routine daily nursing care and services aligned with each resident's plan of care and personal preferences.

  • Advocate for residents' needs and contribute to a safe, supportive, and homelike environment.
