Job description

Medical Coder

The Mann-Grandstaff VA Medical Center (VAMC) has a requirement for medical coding of inpatient, outpatient, and observation encounters. The contractor is responsible for providing accurate and complete medical coding services in accordance with VA guidelines, ICD-10-CM/PCS, CPT, and HCPCS coding standards, and must maintain an overall coding accuracy rate of 95% or above. Accuracy is paramount, and the Coder must maintain strict adherence to VA national policy, local VAMC policy, and applicable federal regulations governing coding of protected health information.

Primary Responsibilities

• Maintain an overall coding accuracy of 95% or above across all record types.

• Perform coding in accordance with current Official Guidelines for Coding and Reporting.

• Code all diagnoses and procedures to the highest degree of specificity.

• Complete provider queries in a timely and compliant manner when required.

• Meet coding productivity standards: Observation - Non-Billable 4 per day; Observation - Billable 3 per day; Inpatient - Billable 3 per day; Inpatient - Non-Billable 5 per day; Outpatient/Recurring/Pro Fees 30 per day; Surgery 7 per day; Prosthetics 30 per day.

• Locate Financial Identification Numbers (FINs) and accurately link documents to the correct veteran record in Cerner/Oracle; immediately flag and report mismatches to the COR.

• Complete 100% internal QA on all records prior to submission.

• Report all errors or discrepancies to the HIMS Supervisor or designee immediately upon discovery, regardless of error ownership.

• Strictly follow VA national policy, Mann-Grandstaff local policy, and HIMS Standard Operating Procedures, including VHA Handbook 1907.01.

Minimum Qualifications

• Active coding credential required (RHIA, RHIT, CCS, CPC, or equivalent).

• Prior experience with VA or federal healthcare coding strongly preferred.

• Demonstrated knowledge of VA-specific coding requirements.

• Knowledge and understanding of medical clinics and clinical documentation.

• Ability to locate Financial Identification Numbers (FINs) and accurately link documents to the correct veteran record in Cerner/Oracle.

• Knowledge of computers, specifically Microsoft Office (Word and Excel).

• Must complete VA Cybersecurity/Privacy Training, VA Privacy and Information Security Awareness training, a Non-Disclosure Agreement (NDA), Cerner/Oracle Revenue Cycle System Training, and Cerner/Oracle PowerChart System Training prior to beginning work.
