Job description

Nationwide Children's Hospital seeks a dedicated outreach coordinator to manage patient and family outreach for community providers serving PFK members. You will support population health partnerships, assist quality improvement efforts, and liaise with physicians and staff to implement services. Responsibilities include monthly impact reporting using claims data, Epic, and EMR data; collaboration with QA, Pharmacy, and Provider Relations; and design of outreach materials with NCH marketing.
