Requirements
  • Active Ohio Registered Nurse (RN) license in good standing
  • Minimum 1 year of nursing experience (home health experience preferred, not required)
  • Strong clinical skills in wound care, infusion therapy, and chronic condition management
  • Ability to work independently in a field-based environment
  • Excellent communication, organization, and time-management skills
  • Valid driver's license and reliable transportation
  • Ability to travel within Ross County and surrounding areas
  • Ability to pass required background screening
Responsibilities
  • Provide skilled nursing care to patients in their homes per the physician's orders and plans of care
  • Perform comprehensive patient assessments
  • Deliver wound care, IV therapy, and medication administration
  • Manage PEG tubes, ostomies, drains, and other skilled nursing needs
  • Monitor patient progress and report changes in condition
  • Educate patients and families on care plans, medications, and preventive health strategies
  • Communicate effectively with physicians, caregivers, and interdisciplinary team members
  • Ensure compliance with state, federal, and agency policies
  • Maintain accurate and timely documentation in electronic medical records
