Family Practice & Infectious Disease CNP

PrimaryOne Health is currently seeking a Hybrid Family Practice & Infectious Disease Certified Nurse Practitioner for our Columbus health centers.

Essential Job Responsibilities:

  • To develop and implement a standard care arrangement with each supervising physician, updated annually. In compliance with Section 472343 of Ohio Revised Code.
  • Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care.
  • Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral.
  • Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues.
  • Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatment and/or medications.
  • Maintains legible, accurate, and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals, and consultations consistent with organization's regulatory standards.
  • Facilitates evaluation of records by physician(s), peers, and Quality and Standards according to protocols, and receives and implements constructive directives.
  • Analyzes new knowledge gained from conferences, workshops, professional literature, or "hands-on training" and assimilates this knowledge into clinical practice.

Ryan White HIV/AIDS Outpatient/Ambulatory Health Services

  • Work as part of a multidisciplinary team that provides comprehensive medical care to patients with special focus on HIV (Human Immunodeficiency Virus) treatment and serving the LGBTQ+ community.
  • Work with various staff to diagnose and manage patients' healthcare. The services will be conducted face-to-face and/or telehealth visits.
  • Review medical histories and perform physical examinations.
  • Diagnostic testing, including laboratory testing.
  • Treatment and management of physical and behavioral health conditions.
  • Behavioral risk assessment, subsequent counseling, and referral.
  • Preventive care and screening.
  • Prescription and management of medication therapy.
  • Treatment adherence.
  • Education and counseling on health and prevention issues.
  • Referral to and provision of specialty care related to HIV diagnosis.
  • Occasional travel required.
  • Other duties or special projects as assigned.

Successful candidates must possess an Ohio State certification as a Nurse Practitioner in the appropriate specialty—pediatric, family, adult, and women's health, and shall maintain such certification or eligibility during the term of their employment.
