    Job Summary
    Primary Care Provider Services - Dayton, OH
    Location: On-Site | Dayton VA Medical Center, 4100 W. 3rd St., Dayton, OH 45417 Schedule: Monday-Friday, 8:00am-4:30pm | No federal holidays Employment Type: Full-Time Contract Contract Period: 8/1/2026-7/31/2027 (+ Option Year)
    Primary Care Physician / Physician Assistant / Nurse Practitioner
    Overview: Aptive Resources is seeking Board Certified/Board Eligible Primary Care Providers to deliver on-site outpatient care to Veterans at the Dayton VA Medical Center PACT clinic.
    Primary Responsibilities
    Responsibilities:
    • Evaluate and treat patients; order and interpret labs, imaging, and diagnostic studies
    • Prescribe medications within scope of practice
    • Consult with sub-specialists and coordinate referrals
    • Conduct advanced care planning and end-of-life discussions
    • Document care in VA Electronic Health Record (EHR/CPRS) by end of each encounter
    • Participate in daily team huddles and weekly staff meetings
    • Maintain patient safety standards and report incidents within 24 hours
    Minimum Qualifications
    Qualifications:
    • MD/DO (Physician), PA-C (Physician Assistant), or NP - Board Certified/Board Eligible in Internal Medicine or Family Practice
    • Current, full and unrestricted license to practice in any U.S. state, territory, or commonwealth
    • Current BLS and ACLS certification
    • Current DEA certificate
    • Minimum 1 year clinical experience; VA experience preferred
    • Must pass background investigation (Tier 1/Low Risk); PIV card required
    • Must complete required VA mandatory training prior to start
