Description
A well-established nonprofit organization seeks Social Workers for full-time, temp-to-perm opportunities supporting community-based housing programs in New York City. (There are several full-time positions available.) This role offers the opportunity to build meaningful relationships with individuals while helping promote stability, wellness, and independent living.
The Social Worker will provide direct services within a collaborative, multidisciplinary team environment and play an important role in supporting participants' social, emotional, and practical needs. This position is a 35-hour workweek role based at a New York City program site.
Position Overview
The Social Worker provides individualized support and case coordination services to adults living in community residential settings. The role involves developing trusting professional relationships, assessing needs, coordinating services, and connecting participants with resources that support long-term stability and well-being.
The position offers a balance of independent work and team collaboration, allowing clinicians to exercise professional judgment while receiving ongoing supervision and support.
Key Responsibilities
  • Provide individualized social work services and ongoing case coordination
  • Conduct assessments and develop service plans in collaboration with participants
  • Connect individuals with community resources and supportive services
  • Maintain accurate documentation and reporting
  • Participate in team meetings, supervision, and interdisciplinary collaboration
  • Support participants in achieving personal goals related to stability and wellness

Qualifications
