Job description

Physical Therapist (Home Health)


Eden Health is seeking a compassionate Physical Therapist to provide in-home therapy services to patients within the community. The Physical Therapist will complete patient assessments, develop individualized treatment plans, coordinate care with physicians and interdisciplinary team members, and provide ongoing therapy services focused on improving patient mobility, function, and independence.


Responsibilities:

  • Perform comprehensive patient assessments and develop individualized care plans.
  • Complete OASIS documentation and timely clinical visit notes within the EHR.
  • Provide appropriate physical therapy treatments and home exercise programs.
  • Collaborate with physicians and interdisciplinary healthcare team members regarding patient care.
  • Educate patients and caregivers on treatment plans and safety.
  • Participate in care coordination, case conferences, and discharge planning.
  • Supervise Physical Therapist Assistants (PTAs) as required.


Qualifications:

  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist license in the practicing state.
  • Prior Home Health and OASIS experience preferred.
  • Current CPR certification.
  • Valid driver’s license, reliable transportation, and proof of auto insurance.
  • Ability to pass criminal background check and national sex offender clearance.
