Job description

A Licensed Practical Nurse to provide in-home services to our patients.
  • Provides skilled nursing care ordered by the physician as indicated by the plan of care to all patients on caseload.
  • Review caseload with supervising RN on a regular basis as established by best practice and/or State guidelines.
  • Obtains necessary physician orders and collaborates with the physician as the patient's needs dictate.
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members (e.g. hospice aide, RN, OT, PT, MSW, ST, Dietitian, Chaplain or Hospice Manager)
  • Uses equipment and supplies effectively and efficiently.
  • Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications.
  • Actively participates in Quality management program.
  • Completes accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion.

  • Graduate of an accredited nursing program.
  • Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in state of practice.
  • Valid state driver's license and reliable automobile, current automobile insurance, and be willing to operate personal car necessitated by nature of job.
  • Experience as an LPN/LVN commensurate with one of the following preferred:
  • One year experience in an acute care setting within the last 24 months;
  • Six months home care experience within the last 12 months preferred.
  • Valid driver's license and reliable transportation
  • Proof of automobile insurance (MVR checked)
  • Proof of current CPR certification
  • Able to pass criminal background check and national sex offender clearance
