We are currently seeking an experienced and compassionate RN Nurse Care Coordinator to join our dedicated team at a long-term care nursing facility in Staten Island, NY.

This is an exciting opportunity for a Registered Nurse who thrives in a leadership and coordination role while making a meaningful impact in the lives of residents and their families.

As the RN Nurse Care Coordinator, you will play a key role in overseeing resident care coordination, interdisciplinary communication, discharge planning, and family support while helping maintain the highest standards of care throughout the facility.

Filipino speaking candidates are required for this position.

Key Responsibilities
  • Coordinate and oversee resident care plans alongside interdisciplinary healthcare teams
  • Communicate with physicians, nurses, therapists, residents, and families regarding care updates and treatment plans
  • Assist with admissions, transfers, discharge planning, and care transitions
  • Monitor resident progress and ensure care plans are being followed appropriately
  • Advocate for residents' healthcare needs and overall well-being
  • Ensure compliance with nursing home policies, state regulations, and quality care standards
  • Participate in care conferences and collaborate with department heads
  • Maintain accurate documentation and resident records
  • Help support and educate residents and family members regarding care services and treatment plans

Requirements

  • Active RN License in New York State
  • Previous experience working in a nursing home or long-term care facility preferred
  • Experience in care coordination, case management, MDS, admissions, or supervisory nursing roles is strongly preferred
  • Must be fluent in Filipino and English
  • Strong organizational and communication skills
  • Ability to multitask and work effectively in a fast-paced healthcare environment
  • Compassionate, professional, and resident-focused attitude
  • Reliable and team-oriented
