Outpatient Physical Therapist
Job Description
The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. Responsible for delegation and direction of all discipline-specific therapy services for those assigned patients, the therapist ensures the design of functional treatment programs that allow for effective return to community activities. Additionally, the therapist serves as an educator for patients, families, students, coworkers, and themselves while performing professional duties associated with the care of patients and the development of therapy services.
Responsibilities
  • Complete comprehensive evaluations of assigned patients and utilize clinical reasoning to develop appropriate treatment plans and goals focused on progressing patients towards discharge.
  • Ensure medical record documentation is clear, concise, accurate, and timely, in line with state and federal guidelines.
  • Participate in student programs as assigned, providing a positive learning environment and supervising students appropriately.
  • Create an individualized professional development plan and pursue ongoing professional growth through education and involvement in professional organizations.
  • Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
  • Understand and apply organizational performance improvement initiatives to enhance the quality of services provided.
Required Experience / Certifications / Licensure
  • Degree in Physical Therapy from an accredited school.
  • 1+ years of outpatient clinical experience.
  • Active CPR certification.
  • Valid state licensure as a Physical Therapist.
Schedule
This position operates on a 5x8.0 shift schedule.
