Summary:
AS A SOCIAL WORKER, you will be responsible to assist patients and families in planning for post-hospital care, you will perform social assessments for discharge planning and develop and maintain files on community resources.

QUALIIFICATIONS:
-Bachelor's degree (B. A.) from four-year college or university in social work or a closely related human services field and a minimum of one year related experience in a social agency or organization dealing with health and/or welfare required
-Ability to handle extreme emotional circumstances of patients required
-Successful completion of fims within six months of start date if working in rehabilitation center
