Minimum Qualifications:
  • Must be a licensed Registered Nurse in the state of Oregon
  • Passionate about helping seniors live their best life

Preferred Qualifications:
  • Experience working in Home Health

Responsibilities:
  • Develop care plans for the patient
  • Complete initial assessment for care of the patient
  • Provide case management from admission to discharge

Care is provided under a physician's order and coordinated through quality practice, case management, and through all aspects of the nursing process.
