About Us
Welcome to Dr. Chakeer's Office. We are a busy, fast-growing medical weight management clinic dedicated to helping patients achieve their health and wellness goals. We pride ourselves on providing compassionate, evidence-based care utilizing the latest FDA-approved medical management strategies. We are looking for a dedicated and empathetic Physician Assistant (PA) or Nurse Practitioner (NP) to join our dynamic team—working alongside Dr. Chakeer and our current, established Nurse Practitioner—to make a real difference in our patients' lives.

The Role
As a PA/NP at our clinic, you will work directly alongside Dr. Chakeer and our existing NP to provide exceptional, continuous patient care. Our clinical workflow is highly collaborative: Dr. Chakeer personally conducts all initial evaluations and establishes each patient's comprehensive, personalized weight management plan. You will then step in to take the lead on the patient's ongoing care, monitoring their progress, managing their medications, and guiding them through their weight loss journey.

Key Duties & Responsibilities
Provide ongoing, high-quality care and follow-up for patients after their initial evaluation and treatment plan establishment by Dr. Chakeer.

Manage and adjust prescriptions for FDA-approved anti-obesity medications (such as GLP-1s) in accordance with the established care plan and clinic protocols.

Order, review, and interpret follow-up laboratory tests and diagnostic studies relevant to metabolic health.

Provide continuous education, counseling, and encouragement regarding nutrition, exercise, and behavior modification.

Monitor patient progress through regular clinic visits, adjusting treatment approaches as needed in collaboration with Dr. Chakeer.

Maintain accurate, timely, and detailed patient records within the Electronic Medical Record (EMR) system.

Collaborate effectively with Dr. Chakeer, our current NP, administrative staff, and medical assistants to ensure seamless patient care.

Qualifications & Requirements
Active, unencumbered license as a Physician Assistant or Nurse Practitioner in the state of Illinois.

Valid DEA registration (or ability to obtain one).

Experience in weight management, endocrinology, internal medicine, or family practice is highly preferred.

Excellent interpersonal and communication skills, with a non-judgmental and empathetic approach to patient care.

Strong clinical decision-making skills, a collaborative mindset, and the ability to thrive in a high-volume clinical setting alongside fellow providers.
