Overview:

The Wellness Assistant will manage a list of member files that require initial outreach, ensure member's receipt of standard disease and wellness education, assist members with creating Healthy Tomorrows by Hines (HTbH) accounts, answer questions on member accounts, assist the wellness program manager with creating educational programs and completing administrative tasks as requested on the wellness mobile/online application and perform other duties as assigned by the program supervisor.


Key Responsibilities

  • Collaborate with      wellness program manager, health coaches, chronic condition nurses, and      other team members to support participant success
  • Perform general      administrative duties 
  • Assist with onboarding      new patients and program participants
  • Support chronic      condition management programs through participant outreach and engagement
  • Provide assistance with      web and app initiatives of wellness and educational programs
  • Maintain accurate      participant records and documentation
  • Respond to participant      questions and provide excellent customer service

Minimum Qualifications Required

  • High school diploma or      equivalent
  • Strong communication and      interpersonal skills
  • Excellent organizational      and time-management abilities
  • Ability to manage      multiple priorities

**Wellness or healthcare experience is preferred, but we welcome candidates from diverse backgrounds who are eager to learn and make a difference.


Physical Requirements:

No significant physical exertion required, however occasional lifting of paper or boxes up to ten pounds.
