Overview
Responsible for performing all duties necessary to provide for the effective daily operation of all departmental services and programs. Responsible for identifying potential and existing drug related problems and taking appropriate actions to prevent or resolve them. Serves as the medication expert on the health care team and takes an active role in the care of patients. Provides education to the community, other health care providers, pharmacy interns, and pharmacy residents. Maintain/enhance pharmacotherapeutic skills through the reading of primary literature, national guidelines and/or medical references Performs related duties as assigned.

What You'll DoClinical Pharmacy Practice
- Review, verify, and process medication orders accurately and efficiently.
- Identify, evaluate, and resolve medication-related issues, including:
- Drug interactions and incompatibilities
- Allergies and contraindications
- Therapeutic duplications
- Dosing concerns
- Pharmacokinetic monitoring
- Anticoagulation management
- Potential inappropriate medication use
- Provide evidence-based drug information and recommendations to healthcare providers.
- Counsel patients and caregivers to promote safe and appropriate medication use.
- Participate in departmental clinical initiatives and therapeutic programs.
Patient Safety & Medication Management
- Ensure timely and accurate medication dispensing and order verification.
- Communicate effectively with providers regarding medication concerns and recommendations.
- Document interventions and outcomes appropriately within the patient record.
- Help maintain formulary compliance through:
- Therapeutic interchanges
- Management of non-formulary medications
- Documentation of formulary interventions
Education & Leadership
- Serve as a pharmacy resource for healthcare teams, patients, and families.
- Support the development and training of pharmacy employees, students, residents, and interns.
- Provide educational presentations, projects, or in-services annually.
- Maintain current clinical knowledge through continuing education, clinical literature, and national guidelines.
Collaboration & Teamwork
- Partner with multidisciplinary healthcare teams to deliver exceptional patient care.
- Foster a culture of professionalism, respect, and continuous learning.
- Participate in special projects, committee support, quality initiatives, and departmental improvement efforts.
- Assist with regional hospital order verification in accordance with established policies and procedures.
QualificationsRequired Education
- Doctor of Pharmacy (PharmD) degree or
- Minimum of three (3) years of Staff Pharmacist experience
Licensure
- Current Missouri Pharmacist License
Certifications
- Missouri Board of Pharmacy Medication Therapy Services Certificate within 90 days of hire
- ACLS certification (or required internal competency modules) within 6 months of hire
