Job description

Facilitates resource management activities for inpatients based on identified patients. Coordinates patient’s transition from hospital to home through collaboration with physicians, nursing staff and other healthcare professionals to enhance continuity of patient care, promote cost effective utilization of resources and enhance patient, nurse and physician satisfaction.

Provides psychosocial assessment, resources and intervention to hospitalized patients.


Education:  Require BS degree in Social Work from an accredited college or university and current Alabama licensure.

Experience:  Two years hospital experience preferred. Comparable combination of education and experience will be considered.  Previous discharge planning or hospital based social service experience very beneficial.

Additional Skills/Abilities:  Social Workers are expected to demonstrate excellent communication skills, to be capable of working with a diverse population and to behave ethically and professionally.  Social Workers interact with professionals from many different social service and governmental agencies, so must be able to collaborate well with others.  Because social work frequently involves crises, the social worker must demonstrate emotional resilience and stability.
