What Youโ€™ll Be Doing:

ยท Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines

ยท Plan nursing interventions for each resident based on individual needs and monitor the implementation

ยท Evaluate the effectiveness of care interventions, identify problems and trends and develop alternative interventions.

ยท Conduct family and resident education

ยท Administer medications and treatments timely and according to center policy

ยท Transcribe physicianโ€™s orders and chart progress notes in an informative, factual manner that reflects the care administered as well as the residentโ€™s response

ยท Identify and report changes in condition to supervisor, physician and family

ยท Accurately identify skin changes and follow skin management protocols

ยท Order and report diagnostic tests timely

ยท Follow Pharmacy policy and procedures for ordering and delivering medications.

ยท Coordinate residentโ€™s rehabilitation program with therapists to ensure rehabilitation needs are met

ยท Monitors that CNA documentation is completed daily

ยท Ensures CNA assignments are completed on time

ยท Perform additional nursing care responsibilities as required and based on State Nurse Practice Act.

ยท Active state RN license

ยท CPR/BLS Certification

ยท Experience in Long Term Care or Skilled Nursing preferred
