Registered Nurse

We're seeking a Registered Nurse (RN) to join our team in a dynamic hybrid, case management role supporting individuals in both Residential and Day Programs. In this position, you'll coordinate and oversee medical services, ensuring each individual receives high-quality, person-centered care.
The RN will play a key role in assessing health needs, managing care plans, and collaborating with interdisciplinary teams, while also providing training and guidance on health-related topics to both individuals and staff. This is an excellent opportunity for a nurse who enjoys a balance of direct support, coordination, and education in a flexible work environment.

FLSA: Exempt

Reports To: Supervising R.N.

Physical Requirements: Walking, bending, pushing, pulling, climbing stairs, lifting, etc.

Hours of Work: Full time, 37.5 hours per week Monday thru Friday. Rotating on-call, evenings, weekends and holidays, as needed. Shifts and hours may vary depending on agency need. Hybrid Remote Schedule working 3 days from home.

Job Summary:

The overall responsibilities of the RN position focus on providing medical services to individuals in Residential and Day Programs. The RN is also responsible for providing training and instruction in health areas as requested to individuals and staff.

Job Qualifications:

  1. Licensed RN in New York State.
  2. One-year clinical nursing experience preferred.
  3. Strong organizational skills and excellent interpersonal skills are very important.
  4. Valid NYS driver's license that meets agency standards and reliable transportation.
  5. Travel is required.
  6. Must be able to read, write and speak the English language.

Major Responsibilities:

  1. Provide medical services to individuals in the residential programs, in compliance with agency, local, state and federal regulations.
  2. Assess for actual or potential health hazards, whether physical or biological, for each individual on an ongoing basis.
  3. Provide health and safety training to agency personnel, to include residential and work sites.
  4. Record and maintain individual medical records in compliance with all federal, state, local and agency regulations, policies & procedures.
  5. Attend and participate in team meetings and surveys as part of the individual's overall plan of care.
  6. Responsible for assuring all communication is accurate and timely, and conveyed to educate and direct care system with instructions as needed.
  7. Pursue personal and professional development and attend mandated agency training as required.
  8. Provide additional caseload coverage as assigned.
  9. Residential and Day visits weekly where there are two or more individuals and more frequently as medically necessary.
  10. Other duties as assigned.
