Key Responsibilities

Patient Population / Client Focus

Your primary focus will be on serving patients in hospice care, including individuals facing life-limiting illnesses. You will work closely with clients and their families to provide emotional support, facilitate discussions about care preferences, and guide them through the complexities of end-of-life planning. Your role is essential in ensuring these clients receive compassionate and dignified care, addressing not only their psychological needs but also their social circumstances through a multidisciplinary approach.

Qualifications
  • A Master's degree in Social Work from an accredited institution is mandatory.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW) is required to practice.

Compensation and Benefits
