Key Responsibilities

• Conduct thorough assessments to evaluate clients' social, emotional, and environmental challenges, ensuring a comprehensive understanding of their needs.
• Develop customized service plans that prioritize actionable goals, clear interventions, and ongoing progress monitoring to support our clients effectively.
• Provide empathetic counseling and support through individual and family sessions that focus on coping strategies, emotional resilience, and skill-building.
• Foster collaborative relationships with clients to help them access community resources and support services that align with their needs and aspirations.
• Implement crisis intervention strategies and develop risk assessment plans to safeguard client wellbeing during urgent situations.
• Engage actively with community agencies and organizations to facilitate coordinated care, advocating for clients' rights and needs.
• Participate in community outreach initiatives aimed at raising awareness of available services and promoting agency programs.
• Contribute to case conferences, team meetings, and participate in training sessions to enhance both personal and organizational knowledge.

Patient Population / Client Focus

Your primary focus will be on serving patients in hospice care, including individuals facing life-limiting illnesses. You will work closely with clients and their families to provide emotional support, facilitate discussions about care preferences, and guide them through the complexities of end-of-life planning. Your role is essential in ensuring these clients receive compassionate and dignified care, addressing not only their psychological needs but also their social circumstances through a multidisciplinary approach.

Qualifications

• A Master's degree in Social Work from an accredited institution is mandatory.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is required to practice.

• Demonstrated skills in effective communication, cultural competence, and adept problem-solving.

Compensation and Benefits

This position offers a competitive salary range of USD 60,000 to 100,000 annually, commensurate with experience and qualifications. In addition to the base salary, we provide a sign-on bonus to new hires, recognizing the value of your expertise in social work.
