Position Overview

As a Remote Licensed Clinical Social Worker (LCSW) at Healthy Life Reformation & Counseling Center, you will play a pivotal role in providing high-quality mental health services to clients in a hospice setting. Work Environment

This role will primarily be remote, allowing for flexible scheduling that accommodates both client and therapist needs. You will collaborate with a committed team of healthcare professionals, including psychiatrists and psychologists, ensuring a comprehensive approach to client care. Key Responsibilities
  • Conduct thorough psychosocial assessments to understand clients' mental health concerns, strengths, and available resources.
  • Deliver individual, family, and group therapy sessions employing evidence-based therapeutic modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-focused therapy.
  • Develop and implement tailored treatment plans that include clear goals and interventions, ensuring the best possible outcomes for clients.
  • Collaborate actively with the interdisciplinary treatment team to create and adjust client-centered care plans that are reflective of individual needs and progress.
  • Provide immediate crisis intervention, risk assessment, and safety planning as necessary, ensuring clients have a safety net during critical times.
  • Maintain meticulous and timely documentation that aligns with ethical and legal standards, contributing to overall case management and service delivery.
  • Facilitate psychoeducation classes and support groups to enhance community understanding and coping strategies for various mental health challenges.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited program.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW).
  • A minimum of two years of experience in a mental health or outpatient setting.
  • Strong understanding of evidence-based therapeutic approaches and a commitment to continuous learning.
  • Exceptional interpersonal, communication, and assessment skills that enable effective and compassionate client interactions.
