Qualifications:

  • Graduate from a physical therapist curriculum approved educational institution

  • Currently licensed in the state of Montana

  • 1 year home health experience, preferred

  • BLS/CPR required for this position

  • Knowledgeable with healthcare regulations, HIPAA compliance, and patient privacy requirements

  • Knowledge of the local healthcare market and community outreach strategies

  • Knowledge of appropriate software including Microsoft Word, Email, Outlook, and Wellsky (EHR)

  • Clean driver’s license with a clean driving record

Principal Responsibilities:

  • Understands and adheres to established policies and procedures

  • Provides physician-prescribed physical therapy

  • Improves or minimizes residual physical disabilities of the patient

  • Returns the individual to optimum and productive level within the patient's capabilities

  • Participates with all other home care personnel in patient care planning

  • Directs and supervises personnel as required

  • Responsible for initial assessment, plan of care, maintenance program development, and modifications and reassessments every 30 days

  • Performs all skilled procedures as ordered by the physician

  • Consult with physicians regarding changes in treatment

  • Writes reports to physicians regarding patient's progress

  • Instruct patients and family/significant others in-home programs and activities of daily living

  • Participates in in-service programs and presents in-service programs as assigned

  • Participates in Quality Assurance and Performance Improvement activities as assigned

  • Attends all patient care conferences as scheduled

  • Prepares clinical and progress notes

  • Assists physician with evaluating level of function

  • Helps develop the plan of care and revise it as necessary

  • Consult with family and Agency personnel

  • Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition under Agency defined time frames

    • Mileage reimbursement.

    • Monday to Friday

    • Weekends as needed
