What You'll Do:

As a Nurse Practitioner at CleanSlate, you'll play a vital clinical role in treating patients with opioid and alcohol use disorders through medication-assisted treatment (MAT). You'll work alongside a team of dedicated medical and behavioral health professionals to ensure high-quality, patient-centered care.

Key Responsibilities:

  • Provide medication-assisted treatment using buprenorphine, Vivitrol, Sublocade, and Brixadi
  • Evaluate patients and determine individualized treatment plans
  • Monitor patient progress and adjust medications as needed
  • Collaborate with physicians and care teams on complex cases
  • Maintain accurate documentation in the EMR within 48 hours
  • Participate in clinical meetings, training, and quality improvement efforts
  • Support telemedicine and occasional regional coverage as needed
