Contract Objective: Provide in-home and/or on-site individual, family and group therapy; complete assessment tools, create individualized service recovery plans in order to assess continuum of care needs.
Requirements:

  • Masters or Doctoral Degree in Social Work, Psychology, or Counseling - in one of the behavioral or social sciences that is primarily psychological in nature, and documentation of supervised clinical experience, or those licensed in Georgia to practice independently, or currently under a licensed professional supervision.
  • Must have at least 1-4 years work experience in counseling and DSM-V diagnosis, to include crisis management, intensive case management, individual and/or family therapy, skill training, and other rehabilitation supports preferred
  • Good verbal and written communication skills.
  • Excellent interpersonal skills and team player concepts.
  • Must be willing to provide community services in Henry, Dekalb, or Clayton counties
  • Must be willing to see children and adolescents in their homes or community.
  • Must have knowledge about BIRP note documentation and clinical documentation
  • Must have knowledge of Cognitive Behavioral Therapy techniques and family therapy techniques
Primary Responsibilities:

  • Therapist and/or S/T will perform individual, family and group therapy encounters as authorized with identified consumers for purpose of assessing status and continued need for child and adolescent services, to include updated treatment plan summaries as needed.
  • Therapist and/or S/T will ensure documentation is inputted into EMR System on a daily basis.
  • Therapist and/or S/T will complete discharge summary on all assigned consumers 7 days prior to end of authorization period.
  • Therapist and/or S/T must attend all mandatory meetings/trainings for task assignments and updates.
