Job Description
Sun River Health provides the highest quality of comprehensive primary, preventative and behavioral health services to all who see it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Health Coach. This position is primarily remote but may require travel to local SRH locations for trainings and or meetings as needed.
The Health Coach provides self-management education and support to patients with chronic medical and behavioral health conditions, including addressing social determinants of health concerns.
ESSENTIAL FUNCTIONS
  • Outreaches and engages high risk patients regarding their engagement with primary care.
  • Facilitates completion of self-management needs assessment with high-risk patients, including the social determinants of health screening tool.
  • Develops a self-management care plan and goals with patients. Sessions are conducted in person or via tele-video (in accordance with NYS regulations). Provides each patient with a copy of their approved plan.
  • Implements self-management care plan with educational session(s). Sessions are conducted in person or via tele-video (in accordance with NYS regulations).
  • Ensure providers have approved and signed all self-management plans.
  • Provides linkage, coordination with, referral to and follow-up with appropriate ongoing service providers, including mental health and medical specialists, utilizing Unite Us and Referral Point.
  • Documents notes and care plans timely, and with accuracy, in agency EMR (eCW).
  • Tracks/ monitors patient progress and produces/maintains detailed, accurate and timely case notes documented in agency EMR.
  • Provides ongoing engagement with patients, at a minimum of two times per year.
  • Other duties as assigned.

Required Education and Skills:
  • High School Diploma / GED required
  • Bachelors Degree is preferred

Required experience:
  • 2+ years relevant experience in healthcare is preferred
  • ECW experience preferred
