Chase Center Rehabilitation and Nursing Services – Logansport, IN
Full-Time

Chase Center Rehabilitation and Nursing Services is hiring Certified Nursing Assistants (CNAs) to join our compassionate care team. CNAs play an essential role in supporting residents with daily care needs, ensuring comfort, dignity, and safety in our skilled nursing environment.

Pay
  • $17–$24.50 per hour

  • Shift Differentials:

    • +$2.50/hour (Evening & NOC)

    • +$3.00/hour (Weekends)

    • Evening/NOC on weekends = +$5.50/hour

Available Shifts

Full time Evenings or Nights - 8hr shifts

Responsibilities
  • Provide hands-on care to residents following individualized care plans

  • Assist with bathing, dressing, grooming, hygiene, and mobility needs

  • Ensure residents are out of bed and engaged per care plan requirements

  • Support residents with therapeutic, neuro/dementia, dialysis, IV therapy, infectious disease, and wound care needs (as applicable)

  • Work collaboratively under the direction of the Charge Nurse

  • Communicate respectfully with residents, families, and facility staff

  • Maintain a safe and supportive environment for all residents

Qualifications
  • High school diploma or GED preferred

  • Must be at least 18 years old

  • An active Indiana CNA certification is required

  • Must provide certification documentation at the time of application
