What You’ll Do

The Clinical Dietitian Diabetes Educator will serve as the go-to resource for diabetes education while also providing comprehensive dietitian services across our hospital and clinics. In this role, you’ll make a direct impact on patients’ health journeys—from acute inpatient care to long-term outpatient management. You’ll have the opportunity to build meaningful relationships with patients across a variety of settings and be a trusted educator and advocate in their long-term health.

  • Serve as the primary diabetes educator, providing individualized diabetes self-management education and skills training to patients of all ages, from pediatric to geriatric, in alignment with ADCES guidelines.
  • Partner with patients and families to support lifestyle changes, prevention of complications, and long-term health outcomes through one-on-one and group education.
  • Provide nutrition services and medical nutrition therapy for hospitalized patients and outpatients, ensuring care aligns with patients’ medical goals.
  • Perform nutrition assessments, create care plans, monitor progress, and provide education and counseling in multiple settings.
  • Write orders for diets, supplements, and enteral/parenteral nutrition per facility policy, and monitor related laboratory data.
  • Collaborate closely with physicians, nurses, and other clinical staff as an integral part of the care team.
  • Travel between hospital and clinic locations to meet patient and program needs.
  • Support performance improvement initiatives and ensure compliance with state, federal, and accrediting agencies.
  • Occasionally work flexible hours, including on-call, evenings, or weekends, as needed.

Who We’re Looking For

  • Maintains Registered Dietitian (RD) status including continuing professional education per the Professional Development Portfolio of the Commission on Dietetic Registration.  
  • Certified Diabetes Care and Education Specialist (CDCES) status with the National Certification Board of Diabetes Educators required within 1 year of beginning the role or mentorship under accredited clinic. Maintains CDCES with throughout GRHD employment. 
  • Must have current BLS card within 3 months of initial employment and maintain current status throughout GRHD employment.
  • Prefer two years’ or more professional experience in a clinical or outpatient setting in a similar role.
