Job description

Registered Dietician/RDN/CDCES

The Bedford-Stuyvesant Family Health Center (BSFHC) is a Federally Qualified Health Center (FQHC) that serves all of the primary health care needs of families in the heart of North and Central Brooklyn. Our mission is to provide the most professional, courteous and highest quality health care, with dignity, to those we serve, especially the undeserved population, without regard for ability to pay.

We are looking for a Full-time Registered Dietician (RD/CDN/CDCES) to promote healthy eating habits through individual nutritional assessment, diagnostic therapy, and counseling services, to educate patients and their families with or at risk for diabetes on self-management. Collaborating with multidisciplinary teams, you will perform in-depth motivational interviews to identify prospective and fully-fledge diabetes; develop a treatment program based on evidence-based procedures and regulatory requirements and monitor adherence. You will also train and educate nursing staff; attend meetings, in-service training, and outreach activities; and participate in quality assurance/improvement monitoring and reporting initiatives. MUST OBTAIN CDCES WITHIN ONE YEAR OF EMPLOYMENT

Responsibilities include but not limited to:

  • Perform individual nutrition counseling, teaching diabetes group classes, assisting with maintaining American Diabetes Association program recognition and participating in continuous quality improvement projects, among other duties.
  • Educate patients on the necessary lifestyle changes involved in diabetic diagnoses including nutrition. Provides patient support between sessions and on an ongoing basis via the Healthy Interactions digital engagement and coaching platform (i.e., map4healthยฎ)
  • Provide ADA Education Recognition Program continuous quality improvement reports as needed and determined by Program Coordinator
  • Develop and Manage the Diabetes program.

Education, Licenses and/or Certifications, Experience Requirements:

  • Master's Degree in Food & Nutrition from an accredited institution
  • RDN (Register Dietitian Nutritionist)
  • CDCES (Certified Diabetes Care and Education Specailist)
  • 1+ Years of experience.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Able to lead and effectively facilitate groups
  • Strong interpersonal, organizational, verbal and written communication skills.
  • Ability to develop a collaborative, therapeutic alliance with individuals
  • Proficient use of EMR, MS Tools including Outlook, Excel, Word and PowerPoint, use of Internet and e-mail, web conferencing.

Salary: Starting $85,000 and based on experience
