Position Summary

The Registered Dietitian/Health Coach utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members in decisions related to achieving and maintaining optimal health status. Appointments via telehealth or virtual only.

This is a Work From Home position. The hours are M-F with one late night until 9pm EST each week.

Must have CDCES.( Certified Diabetes Care and Education Specialist)

Conducts comprehensive evaluation of member's health status and risk with use of clinical tools and information/data review; has knowledge and experience of lifestyle risk behaviors. By assimilating health assessment data analysis, establishes a plan of care based on member's health status/risks, education, and support needs. Identifies quality of care issues through established channels. Utilizes influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health. Utilizing interviewing and active listening skills, provides coaching, in and support to empower the member to make ongoing independent healthy lifestyle choices. Develops and monitors established plans of care, in collaboration with the member and/or attending physician, to meet the member's goals. Utilizes internal policy and procedure in compliance with regulatory and accreditation guidelines.

Required Qualifications

Certified Diabetic Educator

Registered Dietitian with licensure.

3+ years of coaching experience required

Require Bachelor's Degree in health related field (e.g., Dietetics, Health Education, Wellness, Health Promotion) .

Experience in wellness related areas such as diabetes, weight management, stress management, nutrition, physical activity or health lifestyle behavior change.

Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgment in challenging situations.

Preferred Qualifications

Health Coach Certification or Fitness certification recommended. Background in motivational interviewing, active listening skills, Transtheoretical Model, mindfulness based and other evidence-based coaching techniques preferred.

Excellent computer skills including MS Office applications such as Excel, Word, PowerPoint and Outlook.

Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgment in challenging situations.

Ability to build relationship with wellness champions and build a culture of health.

Pay Range

The typical pay range for this role is:

$46,597.00 - $101,228.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
