Job description

Position Objective:
At Luminis Diabetes Center, we are dedicated to improving the lives of individuals living with diabetes through comprehensive education, personalized care, and innovative treatment approaches. Our mission is to empower patients with the knowledge and tools necessary to manage their condition effectively and achieve optimal health outcomes. We value compassion, collaboration, and excellence in care delivery as we strive to be a leader in diabetes education and management. The educator will be responsible for individual and group teaching, regulatory and quality reporting requirements, and the coordination of other Diabetes Center activities and services. The educator works in a collaborative management structure with our Executive Medical Director, the Lead Certified Diabetes Care and Education Specialist (CDCES), the inpatient Certified Diabetes Care Education Specialist Team, and the Administrative Team.
Position Summary:
We are seeking a highly motivated and experienced Certified Diabetes Care Education Specialist (CDCES) to join our interdisciplinary team. The ideal candidate will play a pivotal role in providing patient-centered diabetes education, support, and care coordination to individuals diagnosed with diabetes and related metabolic disorders. This role demands a passionate (CDCES) committed to promoting self-management skills, improving patient outcomes, and supporting the overall mission of Luminis Diabetes Center.
Essential Job Duties:

1) Conduct comprehensive diabetes self-management education (DSME) sessions for individuals and groups, tailored to patients' clinical needs and learning styles.
2) Develop personalized care plans in collaboration with physicians, dietitians, nurses, and other healthcare professionals to optimize diabetes management.
3) Provide ongoing assessment and support to patients regarding medication adherence, lifestyle modifications, glucose monitoring, and complication prevention.
4) Utilize evidence-based practices and current clinical guidelines in diabetes education and management.
5) Facilitate behavior change strategies to empower patients to take active roles in their care.
6) Document all patient interactions, education provided, and care plans accurately in the electronic health record system.
7) Serve as a resource for staff, patients, and families on diabetes-related topics and emerging treatment options.
Educational/Experience Requirements:
- Bachelor's degree in Nursing, Nutrition, Pharmacy, or related health field; Master's degree preferred.
- Minimum of 1 year of clinical experience in diabetes education and management, preferably in an outpatient or specialty clinic setting.
- Proficiency in using electronic health records and telehealth platforms.
- Strong interpersonal and communication skills with the ability to educate and motivate diverse patient populations.
- Demonstrated ability to work collaboratively within multidisciplinary teams.
- Commitment to patient-centered care, cultural sensitivity, and ethical practice.
Required License/Certifications:
- Current certification as a Certified Diabetes Care and Education Specialist (CDCES) by the National Certification Board for Diabetes Educators (NCBDE) or equivalent credential.
- Registered Nurse (RN), Registered Dietitian (RD), Pharmacist (RPh), or other healthcare professional licensure as required by state law.
