As the Care Coordinator, Social Worker (CC-MSW) you will be responsible for exemplary cross continuum care coordination, inclusive of psychosocial assessments and interventions. Position: Care Coordinator MSW
Shift: Per Diem, Varied Shifts; ma inly weekends, some weekdays to cover. 16 hours/month + one weekend per month + 1 holiday per year
Experience:
1. At least 5 years of recent clinical experience in a physician's office, clinic, hospital, or emergency services (EMS) setting, or other equivalent experience.
2. Leadership experience a plus.
Education:
Master's Degree in Social Work from CSWE (Council on Social Work Education) accredited program.
Licensures/Certifications:
Licensed Social Worker strongly preferred.
