Full-Time LVN - Hanford Post Acute
Hanford, California | Salary range $28-$30/hr BOA
Join our dedicated team at Hanford Post Acute, where exceptional care meets compassion. We are currently seeking a Licensed Vocational Nurse (LVN) to provide high-quality care and leadership in our skilled nursing facility.
About the Role:
As an LVN at Hanford Post Acute, you will work collaboratively with our care team to deliver compassionate, personalized nursing care to residents. This role involves direct supervision of nursing assistants, administering medications and treatments, monitoring residents' conditions, accurate charting, and assisting in care planning.
Key Responsibilities:
  • Provide direct patient care in accordance with clinical guidelines
  • Supervise and support CNAs and nurse trainees on your shift
  • Administer medications and treatments as prescribed
  • Participate in admissions, discharges, and care planning
  • Ensure compliance with care plans and infection control procedures
  • Maintain accurate and timely charting/documentation
  • Communicate effectively with residents, families, and medical professionals
  • Respond to emergencies and incidents promptly
  • Assist in quality assurance, staff development, and training

Requirements:
  • Valid LVN license in California (unencumbered)
  • CPR certification required
  • Minimum 1 year of experience in a skilled nursing or long-term care setting preferred
  • Strong clinical, organizational, and communication skills
  • Knowledge of PCC (PointClickCare) is a plus
  • Must be in good standing with the State Board of Nursing
  • Able to lift 25 lbs and meet physical demands of the role
