Job Description
Under the supervision of a Registered Nurse, coordinates the Plan of Care for the patient in the home, hospital or other setting. Works cooperatively with other team members in maintaining standards of professional nursing practice and in accordance with age specific needs and within the scope of practice for License Practical Nurse within New York State.

Education And Credentials
Associate of Applied Science(AAS)degree in Licensed Practical Nursing is required. Valid NYS License as a Practical Nurse (LPN) upon hire is required. Valid NYS Drivers License upon hire is required.
Experience
One year of experience in Acute medical/surgical nursing is preferred.
