Mission and Context:

ProMedix Health extends the reach of medical practices by delivering virtual chronic care management for enrolled Medicare patients between office visits. The work is practical, clinical, and consequential: monitor patients remotely, surface risk earlier, and help prevent decline before it becomes a hospital visit. The Director of Nursing will lead the clinical engine behind that promise, guiding RNs, LVNs, MAs, and Care Coordinators who work directly with ordering nurse practitioners and physicians to execute patient care plans, manage chronic conditions, and keep documentation strong enough to support CMS-compliant billing.
Scope and Impact:

The Director of Nursing will supervise and develop the clinical team delivering virtual chronic care management to Medicare patients, recruiting, onboarding, and coaching staff while setting clear expectations for panel capacity, patient engagement, documentation completion, and care management time. This role owns the clinical quality infrastructure including CMS documentation standards, internal chart audits, remediation plans, staff training, and coordination with revenue cycle and billing partners to ensure billed services are supported by defensible clinical records. The Director also serves as the primary bridge between frontline clinical staff and ordering physicians and nurse practitioners—translating patient findings into actionable updates, clarifying orders for the care team, and enforcing escalation protocols. As the clinical team’s super-user for ProMedix’s proprietary virtual care portal and EHR system, the Director will be the first resource for workflow questions, documentation standards, and platform issue escalation.
Expectations for outcomes:

  • Lead RNs, LVNs, MAs, and Care Coordinators with measurable standards for productivity, engagement, documentation, and quality.
  • Maintain audit-ready clinical records across programs, with deficiencies identified through internal audits and driven to closure.
  • Ensure documentation consistently reflects medical necessity and supports submitted claims in partnership with billing, revenue cycle, and compliance stakeholders.
  • Oversee RPM device alert response, establish response-time expectations, and confirm escalation workflows are practiced consistently.
  • Maintain and improve SOPs for patient onboarding, monitoring, care plan development, clinical escalation, and disenrollment.
  • Keep all clinical staff licenses, credentials, and certifications current with zero lapses.
  • Deliver weekly clinical performance reporting to leadership, including key metrics, variances, and recommended actions.
Ideal Person and Requirements:

The ideal candidate brings the credibility of a clinical leader and the discipline of an operator. They understand that documentation is not administrative cleanup—it is central to quality, compliance, billing integrity, and scale.
  • Active California RN license in good standing.
  • BSN required; MSN preferred.
  • Minimum 5 years of nursing leadership experience, including direct supervision of RNs and clinical staff.
  • Clinical background in skilled nursing, acute care, outpatient chronic disease management, or a comparable high-acuity environment.
  • Strong working knowledge of CMS documentation standards and clinical compliance requirements.
  • Demonstrated experience working directly with billing, revenue cycle, or compliance teams to ensure clinical documentation supports claims.
  • Experience setting and enforcing productivity or panel-management standards tied to billed clinical time.
  • Familiarity with EHR systems in care management or value-based care settings.
  • Strong plus: experience with RPM, CCM, CoCM, TCM, telehealth, or virtual care delivery.
This role is not aligned for candidates whose primary background is home health, hospice, or assisted living; candidates seeking remote or hybrid work; or leaders without direct nursing staff supervision and hands-on provider or billing coordination experience.
