Job Summary and Responsibilities

As our Registered Nurse (RN) in Skilled Nursing Care, you will help patients and residents receive direct nursing care and ensure nursing activities are supervised effectively, so they can maintain the highest degree of quality care and cost-effectiveness within our facility. Every day, you will utilize technology, procedures, protocols, and standards specific to your assigned unit while also assuming a leadership role over nursing personnel. You will be expected to perform legible, timely, concise, and accurate documentation, communicate clearly using proper medical terminology, and adhere to policies, procedures, and standards in delivering care, medication, and treatments. You will assist with admissions, discharges, or transfers and identify changes or abnormal signs and symptoms in patients, ensuring prompt follow-up with physicians and responsible parties. To be successful in this role, you must have knowledge of nursing care principles within the RN, LVN, and CNA scope of practice, working knowledge of Title 22, OBRA regulations, and Joint Commission requirements, and the ability to follow instructions, analyze data, coordinate tasks, and manage workload effectively.

  • Utilizes technology, procedures, protocols, standards and interventions specific to assigned unit.
  • Performs legible, timely, concise and accurate documentation according to policies and procedures. Proficient and competent with the computerized documentation system.
  • Communicates clearly and effectively using proper communication etiquette. Uses medical terminology accurately. Includes reporting abnormals or change in patient condition to appropriate personnel in a timely manner
  • When within scope of practice to correct quality concerns do so immediately and report to nursing management team. If not within scope of practice report quality concern immediately to management team for assistance or direction.
  • Adheres to the policies, procedures and standards in the delivery of the planned resident's care in the preparation and administration of medications and treatments as ordered by the Physician daily.
  • Demonstrates clinical ability to identify changes and/or abnormal signs and symptoms of the aged, acutely or chronically ill and initiates prompt follow up with Physician, resident and/or responsible party and with appropriate documentation with 95-98% accuracy and timeliness daily in accordance with the Nursing Practice Act.

Job Requirements

Required:

  • One (1) year of experience in care of the aged, acutely ill, chronically ill, and patients in need of restorative care.
  • Active Registered Nurse (RN) license in the State of California
  • Basic Life Support (BLS) certification - American Heart Association
  • Skills:
    • Demonstrated ability to communicate proficiently both orally and in writing in the English language
    • Knowledge in the principles of nursing care within the RN, LVN and CNA scope of practice
    • Working knowledge of Title 22, OBRA regulations and Joint Commission requirements

Preferred:

  • One (1) year of experience supervising nursing personnel within SNF/LTC

Bruceville Terrace has been providing quality, compassionate care since 1989. Licensed as the 171-bed sub-acute skilled nursing facility of Methodist Hospital of Sacramento, Bruceville Terrace offers care for the elderly, as well as those requiring extended recoveries.

Bruceville Terrace provides skilled nursing, rehabilitative and complex medical services to our patients and residents using an interdisciplinary team approach. Over half of the facility is dedicated to skilled nursing/convalescent care, while the remaining beds are committed to meet the medical and rehabilitative needs of the short-term patient after a stay at the acute care hospital.
